Service Event Forms

If you are planning a service or volunteer event, please be aware that the college requires certain forms to be filled out -- some before the event and some afterwards.


Mandatory Forms (for any event off-campus connected with Saint Mary's College): These three froms must be filled out BEFORE the event occurs.


 


General Liability Form


Each participants must sign a general liability form before she can participate in an event that occurs off-campus. If you are in charge of the event, you will need to fill out the top part of the form to provide information about the event. Once participants sign the form, you should hold on to them until after the event has taken place. 


Off-Campus Travel Form


The Off-campus travel form needs to be filled out *before* an event takes place. You may attach a list of the names of all participants. It should then be dropped off at the Business Office in Le Mans Hall.


 Medical Release Form


Each student attending an off-campus event should fill out one of these forms. Then the project leader takes the forms with her to the event. That way, if anything happens to one of the participants during the event, you will have the necessary medical information at hand. Note: If a participant does not have health insurance, simply note this at the bottom of the form. Not having health insurance *does not* exclude a person from a service event.
