Forms for Community-Based Learning Classes/Events
Each faculty member who teaches an experiential learning course is required to use several forms available through the OCSE.
Experiential Learning Agreement (Form B):
This form is filled out by the faculty member and includes learning outcomes as well as contact information and details from the community partner. You can have the form signed by the community partner yourself, or you can submit it to the OCSE and we will get the form signed. If you have the form signed by the community partner yourself, please make a copy of the completed contract and send it to the OCSE. [Form B1] Note: if your partnership is with an individual or a group of individuals in the community and not with an agency as a whole, you will use Form B2.
Student Contract (Form A):
This form is filled out and signed by any student enrolled in a EL class who will be engaged in a community component over time (e.g. 15 hours of service over a semester with one agency/person). [Form A]
Travel Form for Off-campus Events:
This form must be filled out prior to a one-time event that takes place off campus (field trip, etc.). You may attach a list of all participants if it is for a group event. Turn this form in to Carol Babcock in the Business Office prior to your event or program. [Travel Form]
Medical Information Form:
This form is for your students to fill out and take to their site supervisor. This way the site supervisor has key medical information (emergency contacts, etc.) in case something were to happen to the student while at the site. The form is voluntary, but we strongly recommend using it. [Medical Information]
Medical Release Form:
This is to be used only in the case of a group of students going to an off-campus site for a one-time event *with a faculty or staff member present*. It allows the Saint Mary's employee to respond appropriately in case of an emergency. Note: the form should still be filled out even if the student has no health insurance. Simply have her write "No insurance" at the bottom of the form. [Medical Release]
